profiles: Use 'gexp-input' instead of two-element lists.

* guix/profiles.scm (package->manifest-entry): Use 'gexp-input' instead
  of two-element lists to denote specific package outputs.
  (manifest-inputs): Likewise.
  (profile-derivation)[info-dir]: Likewise.
