TH2M; Extract phase-transition data and model

Dmitry Yu. Naumov requested to merge endJunction/ogs:Th2mPhaseTransitionConstitutiveRelations into master

Extraction of the phase-transition data followed by the corresponding model and final refactorings to avoid data duplication in ip_data and phase-transition data.

Certainly not the most comprehensive model separation and a lot is still to do. Important was the data de-duplication.

Each commit compiles and runs all ctests successfully. Review commit-wise.
