Added size assertions, used algorithms.

Writes output files to the specified output directory.

The post() is split from result output as it is intended
for "after solve" procedures and not output.

Small cleanups.

Removes now empty initializeBoundaryConditions.

 - initializeNeumannBcs() is no longer needed.
 - make _neumann_bcs a vector of unique_ptr's.

The initialization is split into a generic one placed in Process and a specific
one kept in GroundwaterFlowProcess.
